Today’s Theme: Quantity to Dispense

A prescription tells you more than what medication a patient should receive.

It also provides the information needed to determine what quantity of medication is needed to complete the prescribed therapy.

A simple way to think about it is:

Amount per dose × doses per day × number of days = quantity to dispense

Before calculating, identify three things:

  • What quantity is used for each dose?
  • How often is the medication taken?
  • How long will the patient take the medication?

Calculation Corner: Quantity to Dispense

Example 1: Capsules

A prescription reads:

Cephalexin 500 mg capsules

Take 1 capsule by mouth four times daily for 7 days.

How many capsules are needed?

Step 1: Identify the amount per dose

1 capsule per dose

Step 2: Identify the number of doses per day

Four times daily = 4 doses per day

Step 3: Identify the duration

7 days

Step 4: Calculate

1 capsule/dose x 4 doses/day x 7 days

= 28 capsules

Answer = Dispense 28 capsules

Example 2: Oral Liquid

A prescription reads:

Amoxicillin 400 mg/5 mL oral suspension

Take 7.5 mL PO Q12H for 10 days.

How many milliliters are needed to complete therapy?

Step 1: Identify the amount per dose

7.5 mL

Step 2: Identify the number of doses per day

Q12H = every 12 hours = 2 doses per day

Step 3: Identify the duration

10 days

Step 4: Calculate

7.5 mL/dose x 2 doses/day x 10 days

= 150 mL

Answer = Dispense 150 mL

A prescription reads:

Clindamycin 300 mg capsules.

Take 1 capsule PO TID x 7 days.

How many capsules should be dispensed?

Answer

Step 1: Identify the amount per dose

1 capsule

Step 2: Identify the number of doses per day

TID = three doses a day

Step 3: Identify the duration

7 days

Step 4: Calculate

1capsules/dose × 3 doses/day × 7 days

= 21 capsules

Answer:

Dispense 21 capsules


What if the Directions Change?

Not every prescription uses the same amount throughout therapy.

Consider:

Prednisone 10 mg tablets

Take 3 tablets daily for 3 days, then 2 tablets daily for 3 days, then 1 tablet daily for 3 days.

Calculate each part separately.

First 3 days:

3 tablets/day × 3 days = 9 tablets

Next 3 days:

2 tablets/day × 3 days = 6 tablets

Final 3 days:

1 tablet/day × 3 days = 3 tablets

Now add them:

9 + 6 + 3 = 18 tablets

Answer

Dispense 18 tablets.


Common Mistakes

Watch for these:

  • Forgetting to multiply by the number of doses taken each day
  • Confusing the amount per dose with the amount per day
  • Missing the duration of therapy
  • Misinterpreting BID, TID, or QID
  • Forgetting to calculate each portion of a taper separately
  • Using the drug strength when the question asks for the number of tablets, capsules, or milliliters
  • Entering the calculated amount when the product is only supplied in fixed package sizes

Does this quantity provide enough medication for every prescribed dose for the full duration of therapy?

What should you know?

Lisinopril lowers blood pressure by inhibiting angiotensin-converting enzyme. Most ACE inhibitors share the -pril ending (enalapril, ramipril, benazepril, quinapril) which is a fast way to recognize the class on an exam or a medication label.

Safety Check

  • Pregnancy: ACE inhibitors carry a boxed warning for fetal toxicity and are discontinued as soon as pregnancy is detected. Route to the pharmacist.
  • Angioedema: swelling of the face, lips, tongue, or throat requires immediate medical attention.
  • Dry cough: a persistent dry cough is a recognized adverse effect and a common reason therapy is switched.
  • Dizziness: most likely when starting therapy or after a dose increase.
  • Monitoring: blood pressure, potassium, and kidney function are followed during therapy.
